SMS TERMS & CONDITIONS

These SMS Terms & Conditions apply to text messages sent by or on behalf of Phoenix Health and WellBeing, LLC, doing business as Phoenix Health & WellBeing (“Phoenix Health & WellBeing,” “we,” “our,” or “us”).

Program Description

When you provide a mobile telephone number and affirmatively consent to text messaging, you may receive messages concerning:

  • Consultation requests and follow-up;

  • Appointment scheduling, confirmations, and reminders;

  • Office, provider, or service updates;

  • Billing, insurance, or administrative matters;

  • Patient portal notifications;

  • Resources or information you request; and

  • Other service-related communications.

We will send promotional or marketing text messages only when permitted by law and consistent with the consent you provided.

Consent

Consent to receive text messages is not a condition of purchasing or receiving services. Your decision not to consent to text messaging will not prevent you from requesting care. You may contact us by telephone at 480-256-9299 or by another available method.

Message Frequency

Message frequency varies depending on your requests, appointments, and services.

Message and Data Rates

Message and data rates may apply according to your mobile carrier and service plan. Phoenix Health & WellBeing is not responsible for carrier charges.

Opting Out

You may opt out of nonessential text messages at any time by replying STOP. We may send one final message confirming that your opt-out request was processed.

After opting out, you may still receive communications that are otherwise permitted or required in connection with active healthcare services, immediate safety concerns, billing, legal obligations, or other essential matters. When appropriate, those communications may be provided by telephone, patient portal, email, or mail instead of text.

Privacy and Security

Your privacy is important to us. Our collection and use of information through the Website are described in our Website Privacy Policy. Our handling of protected health information is described in our HIPAA Notice of Privacy Practices.

We do not sell or share mobile telephone numbers, text-message opt-in information, or consent records with third parties or affiliates for their own marketing or promotional purposes. We may provide this information to vendors that assist us in delivering text messages, but only as necessary to provide those services.

Standard SMS text messaging may not be fully secure. Please do not send detailed diagnoses, medication information, treatment records, Social Security numbers, payment-card information, or other highly sensitive information by text. Use the secure patient portal when requested.

Emergencies

Text messaging is not monitored continuously and must not be used for emergencies or crisis support.

If you are experiencing an emergency, call 911 or go to the nearest emergency department. If you are experiencing a mental health crisis, call or text 988 to reach the Suicide & Crisis Lifeline.

Carrier and Delivery Disclaimer

Wireless carriers are not responsible for delayed or undelivered messages. Delivery may be affected by your device, carrier, network availability, telephone-number changes, or other circumstances outside our control.

You are responsible for notifying Phoenix Health & WellBeing if your mobile number changes or is reassigned.

Eligibility

You must be the account holder for the mobile number provided or have authorization from the account holder to enroll that number in text messaging.

Changes to These Terms

We may update these SMS Terms & Conditions. Updated terms will be posted with a revised effective date. When required, we will provide additional notice or obtain new consent before making material changes to how text messages are used.
